Learn how you can help reduce medication errors, length of patient stay and hospital costs through the Partnered Pharmacist Medication Charting (PPMC) scaling project at the PPMC forum.
Alfred Health medical, pharmacy, nursing and executive staff will speak at the forum and Safer Care Victoria representatives will also be present.
You will learn:
- about the PPMC model, which was first piloted by Alfred Health in 2015 to address the frequency of medication errors in hospitals and the impact on patients
- how the model's expansion in 2016-17 reduced medication errors and length of patient stay, and saved $1.95 million in total hospital costs
- how the Better Care Victoria Innovation Fund is supporting the expansion of the PPMC model across up to 24 health services in regional and rural Victoria.
There will also be opportunities to network with Alfred Health and Safer Care Victoria representatives over afternoon tea.
